KohlsFeedback.com: The Kohl’s Survey, Your Store Number and Access Code, and the Coupon Question

This page reflects KohlsFeedback.com as it loaded on October 28, 2026: where the address forwards, every field on the first screen, and each link in its footer.

Not run by Kohl’s. myreceiptsays.com is an outside guide with no tie to Kohl’s, Inc. or Medallia. Your store number and access code are meant for Kohl’s survey form, so don’t hand them to anyone else, us included.

Open KohlsFeedback.com

You’ll need two numbers from the bottom of your receipt: the store number and the access code.

Open KohlsFeedback.com

The address forwards to survey3.medallia.com, and the footer links to Kohl’s own privacy policy.

In short: KohlsFeedback.com asks for your store number (up to four digits) and the access code from the bottom of your receipt, numbers only. The first page promises no coupon, prize or drawing, and links no rules. It also warns that a Kohl’s representative may contact you about your answers. Meanwhile, guides that promise a free coupon or a prize draw don’t point to any Kohl’s source.

Does KohlsFeedback give a coupon or prize?

Not according to anything Kohl’s publishes on the survey. The welcome text thanks you for your visit and says your feedback is valuable. In other words, there’s no offer line, no sweepstakes mention and no rules link.

Some guides list a “Free Coupon” with “No Limit” on validity, while others describe a random prize draw. However, neither cites Kohl’s. So if your receipt prints an offer next to the survey invitation, follow the receipt; otherwise, assume the survey is for feedback only.

By contrast, other retailers are clearer about it. Off-price chains publish real drawings, such as the monthly $500 Marshalls card drawing. Burger chains, meanwhile, put the offer on the survey itself, as Burger King’s survey offers a free item for contact details.

Without published rules, there’s no stated deadline either. Still, receipt surveys often expire fast; Dunkin’ gives you three days for its donut offer, for example. Therefore, taking Kohl’s survey within a day or two costs nothing.

Clean up your KohlsFeedback numbers

First, paste or type both numbers as they appear on your receipt, hyphens and all. Then the tool strips everything the form doesn’t accept, checks the store number fits in four digits, and counts the access code. Meanwhile, it runs only in your browser. After all, long codes trip people up everywhere, and Loblaw’s StoreOpinion code runs 19 to 21 digits.

What KohlsFeedback asks for

The instructions read: Using the information found at the bottom of your receipt, please complete the following fields. After that come two boxes, Store Number and Access Code, and a Begin Survey button.

To begin with, the store box stops at four characters, with a counter showing “0 / 4”. In contrast, the access code box has no visible limit. One guide says the code is 16 digits; however, we couldn’t confirm that, because Kohl’s doesn’t state a length. Either way, type digits only, since the page says Do not include hyphens or other characters.

Also, before you start, note the line about contact. By taking the survey, You hereby acknowledge that a Kohl’s representative may reach out to you to discuss your survey responses. Most receipt surveys don’t say that up front; they lead with the reward instead, the way IHOP prints its pancake offer on the receipt.

How to take the Kohl’s survey

  1. First, find the store number and the access code at the bottom of your Kohl’s receipt.
  2. Next, go to www.kohlsfeedback.com; it forwards you to Kohl’s survey on survey3.medallia.com.
  3. Type the store number, up to four digits.
  4. Then type the access code using numbers only, with no hyphens or other characters.
  5. Then press Begin Survey and answer the questions about your Kohl’s visit.
  6. Finally, read the last page and keep any code or offer it shows.

When KohlsFeedback rejects your receipt

  • Store number won’t fit. The box holds four characters. If your receipt shows more, you’ve likely copied a register or transaction number. Box layouts differ by chain; Ross’s five-box receipt number is a good example.
  • Code refused. Remove hyphens and spaces first, because the form accepts numbers only. After that, compare digit by digit with the receipt.
  • No survey prompt on the receipt. For one thing, not every Kohl’s receipt invites feedback, so the codes may simply be missing. Grocery chains work similarly; Kroger’s receipt invitation sets the terms there.
  • Online order with store pickup. Similarly, the survey asks for a store receipt, so an online confirmation email won’t have these numbers.
  • The page won’t load. In that case, use the Survey Support link at the bottom, which goes to Medallia’s help site.

KohlsFeedback claims we couldn’t back up

The guide saysWhat Kohl’s survey shows

“Receipt Valid For: 02 Days (48 Hours)” (survey.contact)No deadline shown
“enter the 16-digit access code” (survey.contact)No length given; the box has no limit

Survey at “www.KohlsListens.com” (several guides)That address didn’t load for us; KohlsFeedback.com is the working one

In short, none of these guides links a Kohl’s document, which is the tell. Surveys with real prizes, on the other hand, publish rules you can read, as Walgreens posts a new rules PDF every month.

Fake Kohl’s survey sites and offers

Search for this survey and you’ll find dozens of lookalike addresses on .online, .store, .cfd and .site domains, many using “kohlsfeedback” in the name. Even so, they aren’t Kohl’s. The working entry point is KohlsFeedback.com, which forwards to survey3.medallia.com. Other big chains use the same platform, including CVS, whose survey also lives on Medallia.

Likewise, nothing on the real survey asks for a card number, Kohl’s Charge details or a password. A message offering Kohl’s Cash or a gift card for “confirming” an account is phishing, and the FTC warns that real prizes never cost money. Retail scams follow the same script, so our Target survey guide covers the genuine Target entry for comparison.

KohlsFeedback without a receipt

There isn’t a route, because the survey needs both numbers from a store receipt, and since there’s no sweepstakes, there’s no mail-in entry either.

Surveys tied to real drawings typically offer a free route, because US sweepstakes can’t require a purchase. For example, Publix accepts mailed entries for its drawing, while Kohl’s simply has nothing to enter.

KohlsFeedback FAQ

Using the survey

Is KohlsFeedback.com legit?

Yes. It forwards to survey3.medallia.com, where the Kohl’s survey links Kohl’s own privacy policy in its footer.

What do I need from my Kohl’s receipt?

You need the store number and the access code, both from the bottom of the receipt. Enter numbers only.

How many digits is the Kohl’s store number?

Up to four. The Store Number box stops at four characters.

How long is the KohlsFeedback access code?

Kohl’s doesn’t say, and the box has no limit. One guide claims 16 digits, so type whatever digits your receipt prints, without hyphens.

Is there a deadline for the Kohl’s survey?

The survey page shows none. Guides claim 48 hours without citing Kohl’s, so take it soon to be safe.

Rewards and privacy

Do you get a coupon for the Kohl’s survey?

The survey page doesn’t offer one. Follow any offer printed on your receipt or shown on the last page.

Is there a Kohl’s survey sweepstakes?

We found none: no prize, sponsor or rules link on the survey page.

Will Kohl’s contact me after the survey?

Possibly, because the first page says a Kohl’s representative may reach out to discuss your survey responses.

Is the Kohls feedback survey at KohlsListens.com the same?

Some guides name it; however, it didn’t load for us. KohlsFeedback.com is the address that works.

Can I take the Kohl’s survey without a receipt?

No. It needs the store number and access code from a receipt, and there’s no mail-in alternative because there’s no drawing.

Where this Kohl’s survey information comes from

Of course, Kohl’s could add an offer or a drawing at any time. If your receipt or the survey’s last page shows one, that beats this page.

Kohl’s, Inc. and Medallia had nothing to do with writing this guide and haven’t signed off on it. The Kohl’s name is the retailer’s trademark, and it appears here just so readers know whose survey is described. Kohl’s own survey pages win over anything on this site.
