Two Publix sources shape this Publix survey guide. One is the survey page, and the other is the current round of Publix Customer Voice Survey Sweepstakes rules. When Publix starts a new round, check the date near the title to see whether this page has caught up.
Independent of Publix. We’re a separate site writing about the Publix survey. That’s why we’ll never request the code on your receipt or any personal details.
PublixSurvey.com: type it into the address bar
The sweepstakes invitation on a Publix receipt points you to PublixSurvey.com. Typing www.publixsurvey.com forwards you to Publix’s survey page on publix.com, the page its official rules name for online entry. Searching for it is how people land on lookalike sites.
www.publixsurvey.com
This goes to www.publixsurvey.com, which forwards you to www.publix.com/pages/customer-satisfaction-survey. That page’s Start the Survey button opens the survey on survey.bellomyonline.com, run by Bellomy Research, the administrator named in the rules.
A Publix spokeswoman told WFTV: The only official Publix customer survey is accessed directly by typing into a browser the full URL printed on the receipt.
If your receipt prints an address, typing it is the safest way in.

The PublixSurvey.com $1000 prize, in short
The Publix receipt survey takes about 10 minutes and needs the invitation’s four-digit store number, 16-digit survey code and the time of your visit. Finishing it enters you in the Publix Customer Voice Survey Sweepstakes: three $1,000 Publix gift cards per quarterly round, one per drawing. You get one entry per round, online or free by mail, and a losing entry carries forward to the round’s later drawings.
What the $1,000 actually is
- $1,000Publix gift card, not cash
- 3 prizesper round, one per drawing
- 1 entryper person or email, per round
- 14 stateswhere you must live to enter
The receipt invitation promises a chance to win $1000 in Publix gift cards
, and the official rules list Prizes (3): $1,000 Publix gift card. Approximate Retail Value (ARV): $1,000.
For each drawing, the administrator will randomly choose one (1) name from all timely entries received.
The gift cards can’t be turned into cash, and a household can win only one.
Nobody wins at the end of the survey. Winners are drawn on the dates in the rules, and odds depend on how many entries come in. Winners get a 1099-MISC form if required and pay any taxes, and Publix can substitute a prize of equal or greater value.
The detail most guides miss: Non-winning entries will carry forward to subsequent drawings, if any.
Enter before the first drawing’s deadline and you’re in all three.

Which drawings will your entry be in?
Pick the day you’ll take the survey or post your mail-in card. The checker reads the drawing table above and counts the carry-forward rule, using Eastern Time, the zone the rules’ deadlines are in. It runs in your browser and sends nothing anywhere.
No receipt? Enter free by mail
The rules say NO PURCHASE NECESSARY TO ENTER OR WIN.
Print or type your name, email address, phone number and date of birth on a piece of paper or an index card, and mail it to:
Attn: Marketing/Research
3300 Publix Corporate Parkway
Lakeland, FL 33811
The first line names the round and changes every quarter, so an old round’s name can miss the current one. The rules allow one mail-in entry per envelope, and a mailed card uses your one entry for the round. Confirm the address in the current official rules before posting.
The store number, 16-digit survey code and timestamp
Publix’s survey page says You will need to have your invitation available to continue.
The survey’s first screen then asks for three things from the front of your receipt:
- Store number: a four-digit number at the end of the sweepstakes invitation, after your itemized receipt.
- Survey code: the 16-digit number directly below the store number. In the example receipt on that screen, the code starts with a zero, so type every digit, leading zeros included.
- Timestamp: the time to the right of the date on the bottom line of the receipt, entered as 00:00. The example shows 24-hour time, such as 17:02.
The screen also has a box marked My receipt does not have a Survey Code
. The survey is offered in Spanish too. Neither the rules nor the survey page gives a deadline for taking it after you shop, so do it while the invitation is recent.
The contact form at the end asks for your first name, state of primary residence and phone number. Email is optional unless you pick it as your preferred contact. The same form has a Prefer not to participate in sweepstakes
box: leave it unticked if you want the entry. You must be above the age of majority in your state, which is higher than 18 in some states.

How to take the Publix survey
- Find the sweepstakes invitation at the end of your Publix receipt, with the four-digit store number and 16-digit survey code, plus the time on the receipt’s bottom line.
- Type www.publixsurvey.com into your browser’s address bar. On Publix’s survey page that opens, choose Start the Survey.
- Enter the store number, the 16-digit survey code with any leading zero, and the timestamp as 00:00. The next screen names the store the survey will ask about, so check it’s yours.
- Answer the questions about that visit without using your browser’s back button. Publix says the survey takes about 10 minutes.
- On the contact form, leave Prefer not to participate in sweepstakes unticked and give your first name, state and phone number. Email is optional unless you choose it as your preferred contact.
- Look for the screen saying you are now entered into the sweepstakes drawing. That’s your one entry for the round, online or by mail.
What the survey asks, screen by screen
We took the survey to see what’s in it. Ratings run from 5 (best, on the left) to 1, and your answers don’t change your odds, since winners are drawn at random.
- How satisfied you were with the visit, and how likely you are to recommend the store
- Ratings for sale prices, value for money, the store’s environment, service, competitive prices, products and employees
- Which departments you visited, from floral and produce to the deli, bakery and pharmacy, then a rating for each
- How you checked out, which employees you dealt with, and ratings for the checkout, the store layout, parking and cleanliness
- How your groceries were bagged, and an optional offer to answer more questions for up to 5 extra minutes
- Where you do most of your grocery shopping, then the contact form

If the Publix survey won’t work
Search shows lookalike sites
Type the address from your receipt into the address bar, or use the publix.com link above. Publix says lookalike sites appear in search results.
The code is rejected
Check you’ve typed the 16-digit survey code, not the four-digit store number above it, and kept any leading zero. The code is all digits. We can’t see why the survey site turns down a particular code.
The timestamp won’t fit
Use the time on the receipt’s bottom line, next to the date, in 24-hour form as printed: 17:02, not 5:02.
You went back a page
The survey asks you not to use your browser’s back button. If it breaks, start again from www.publixsurvey.com.
No survey code on your receipt
The survey’s first screen has a My receipt does not have a Survey Code
box; we haven’t tested what follows it. The free mail-in entry always works.
The store named isn’t yours
After the first screen, the survey names the store it will ask about. If it’s the wrong one, the store number was probably mistyped.
Still stuck
The survey’s last screen lists Publix Customer Care: 1-800-242-1227, publix.com/contact, or P.O. Box 407, Lakeland, FL 33802, ATTN: Customer Care. We can’t fix survey problems from here.
A screen says you’ve won
Close it. Drawings happen on set dates after the deadlines, so an instant win message isn’t the Publix sweepstakes.
What other Publix survey guides get wrong
We checked guides that rank for these searches against Publix’s rules. These mistakes could cost you an entry.
| What the guide says | What Publix’s rules say |
|---|---|
| The sweepstakes runs April 1 to June 30, 2026 (publixsurveyus.com) | That was the Spring 2026 round, which has closed. Each quarter has its own rules and dates. |
| Mail your entry to a Fall 2025 or Winter 2024 address (publixsurveyus.com, publixsurvey.cloud) | The mail-in address names the current round, which changes every quarter. |
| You must be 18 or older (publixsurveyus.com, publixsurvey.online) | Above the age of majority in your state, which is more than 18 in some states |
| One entry per household per entry period, and each new receipt is a fresh chance (publixsurveyus.com) | One entry per person or email address for the round, however you enter; extra entries are subject to disqualification |
| Survey codes expire 7 days after purchase (publixsurvey.online) | Neither the rules nor Publix’s survey page gives a time limit. |
| Non-winning entries don’t carry forward (sweepstakesbible.com) | Non-winning entries will carry forward to subsequent drawings, if any. |
Spotting fake Publix survey sites
WFTV’s Action 9 reported a lookalike Publix survey site that told a shopper he’d won $1,000, then pushed him to buy an Apple gift card. Publix told the station it does not ask customers to provide payment information, purchase gift cards or share financial details
.
Check the address bar: the real survey page is on publix.com, www.publixsurvey.com forwards there, and the survey runs on survey.bellomyonline.com. Several guides put “official” in their titles without being Publix. This site isn’t Publix either, and it never asks for your receipt or details. The survey’s last screen says a winner will receive an email from sweepstakes@publixfeedback.com
, and winners can also be reached by phone. Treat a “you’ve won” email from any other address with suspicion, and anyone asking for a fee, gift cards or bank details is running a scam. The FTC’s advice is the same: real prizes are free.
Publix survey questions
- Is the PublixSurvey.com $1000 prize real?
- Yes, as a drawing. Each round, Publix’s rules give away three $1,000 Publix gift cards, one per drawing, picked at random. Finishing the survey gets you an entry, not a prize.
- Is PublixSurvey.com the real Publix survey?
- Typing www.publixsurvey.com forwards you to Publix’s own survey page at publix.com/pages/customer-satisfaction-survey. Publix told WFTV the safest way in is to type the full address printed on your receipt.
- How many times can I enter the Publix survey sweepstakes?
- Once per round. The rules allow one entry per person or email address, whichever way you enter, and extra entries can be disqualified.
- Can I enter without a receipt?
- Yes. No purchase is necessary: you can mail your name, email address, phone number and date of birth on paper or an index card to the address in the current rules.
- Who can enter the Publix survey sweepstakes?
- Legal US residents of the 14 states listed in the rules who are above the age of majority in their state. Publix employees and their families can’t enter.
- Is there a time limit on the Publix receipt survey?
- Publix’s rules and survey page don’t give one, despite guides that say 7 days. Take the survey while your invitation is recent.
- How many digits is the Publix survey code?
- 16. The survey’s first screen says the survey code is the 16-digit number directly below the four-digit store number, at the end of the receipt’s sweepstakes invitation.
- Do I have to give my email address?
- No. The contact form says email is optional unless you choose it as your preferred contact. Don’t tick Prefer not to participate in sweepstakes, or you’ll opt out of the drawing.
- Do my survey scores affect my chances of winning?
- No. The rules say the administrator randomly chooses one name from all timely entries for each drawing.
- How are Publix survey winners contacted?
- By phone or email from the entry details. The survey’s last screen says winners get an email from sweepstakes@publixfeedback.com, and the rules give three business days to respond.
