DQFanSurvey.com and DQFanFeedback.com: Which Dairy Queen Survey Takes Your Receipt

We compared DQFanSurvey.com and DQFanFeedback.com side by side on October 28, 2026, field by field, because Dairy Queen receipts point to one or the other and the two ask for different things.

Not affiliated with Dairy Queen. myreceiptsays.com works independently of American Dairy Queen Corporation and of SMG, the company hosting both survey sites. Whatever number your receipt prints is meant for the DQ survey only, so don’t send it to us or anyone else.

Open DQFanSurvey or DQFanFeedback

Receipt shows a 19-digit survey code

Open DQFanSurvey.com

Receipt shows a 10-digit phone number or a 15-character code

Open DQFanFeedback.com

Both buttons open the “United States and Canada DQ® Fan Feedback” survey on SMG (Service Management Group) software. Type whichever address your own receipt prints.

In short: DQFanSurvey.com asks for a 19-digit survey code. DQFanFeedback.com instead asks for the 10-digit phone number printed on your receipt or coupon, plus the date and time of your visit; with no phone number, it takes a 15-character code with zeros in place of the letter O. Neither start page names a prize or a deadline. Guides report a coupon for a free Dilly Bar, but only your receipt and the survey’s last page can confirm the offer.

Does DQFanSurvey give you a free Dilly Bar?

Possibly, but the survey doesn’t promise it up front. Neither DQFanSurvey.com nor DQFanFeedback.com mentions a reward on its first page. That’s different from McDonald’s McDVoice survey, which promises a validation code before you start.

SurveyMemo reports a coupon code for a free Dilly Bar at participating Dairy Queen locations, and adds that the reward may vary. So check your receipt for any offer it prints, and follow whatever the last page of the survey tells you. If the receipt is a coupon receipt, its wording comes first. Some chains skip receipt coupons altogether, as Chick-fil-A does by emailing its free-sandwich offer.

Frozen-treat fans may know a similar setup from Culver’s custard survey, where the offer printed on the receipt decides the reward.

Which DQ site takes your number?

Type the number from your receipt exactly as printed. The tool counts it and tells you which site and which box it belongs in, based on the rules each start page states. It runs in your browser only. We built a similar box-splitter for the 15-digit number on Ross receipts.

What DQFanSurvey and DQFanFeedback ask for

On DQFanSurvey.com, the first page says Please enter the 19-digit survey code printed on your receipt. The code splits across three boxes, and nothing else is needed to begin.

DQFanFeedback.com works differently. It first asks for the 10-digit phone number printed on your receipt/coupon, then the date and the time of your visit. Like Taco Bell’s receipt survey, it wants the time as hour, minute and AM or PM, so convert a 24-hour receipt time first.

If your receipt has no phone number, follow the link on that page. It opens a form that asks for the 15-character survey code printed on your receipt and warns: Please do not use the letter ‘O’ when entering your Survey Code. Neither site publishes a deadline, so take the survey soon after your visit. Codes vary a lot between chains: Chipotle uses a 20-digit code, for comparison.

How to take the DQFanSurvey

  1. First, see whether your Dairy Queen receipt prints a 19-digit survey code, a 10-digit phone number or a 15-character code.
  2. Next, go to www.dqfansurvey.com for a 19-digit code, or www.dqfanfeedback.com for a phone number or a 15-character code.
  3. Select Español or Français at the top if you’d rather not answer in English.
  4. Type the code, or the phone number plus the date and time of your visit, using a zero for any letter O.
  5. Then answer the questions about your Dairy Queen visit.
  6. Finally, read the last page and note any coupon or validation code it gives you.

When DQFanSurvey rejects your receipt

  • If DQFanSurvey.com rejects a 10-digit number, you’re on the wrong site; that number belongs on DQFanFeedback.com.
  • When a 15-character code fails, look for a letter O and type a zero instead, as the form instructs.
  • If the date menu won’t show your visit year, the receipt may be too old, since the year menu listed only 2025 and 2026.
  • When a code is refused as used, SMG’s survey terms explain why: a code that has already been used to submit responses can’t start another survey. The same rule applies on Zaxby’s SMG-run survey.
  • If you have no receipt at all, there’s no route in, because both sites need a number from the receipt. Retail surveys can be just as strict; Target’s needs a User ID and Password from the receipt.

Where DQ survey guides get it wrong

The guide saysThe survey says
“your receipt should see a 10-digit survey code” (dqfanfeedback.surveymemo.com)DQFanFeedback.com asks for “the 10-digit phone number printed on your receipt/coupon”
Without it, “you can use the store number, date, and time instead” (dqfanfeedback.surveymemo.com)The fallback form asks for a 15-character survey code, with no store number box
“Receipt Valid For 3 Days” (dqfansurvey.surveymemo.com)Neither start page states a deadline

The free Dilly Bar itself may well be right for many receipts. It just isn’t something either start page promises, which is why we treat it as reported rather than confirmed. We took the same approach with the Wendy’s survey, where guides also describe the reward.

Fake Dairy Queen survey sites

The real survey pages end with © 2026 SMG. All rights reserved. and link to the SMG and DQ privacy policies. Lookalike addresses that add extra words to “dqfansurvey” or “dqfanfeedback” aren’t run by Dairy Queen, and this site isn’t Dairy Queen either.

Also, no DQ survey asks for card details or charges a fee. If a message says you’ve won a DQ prize but must pay first, it’s a scam; the FTC says real prizes never cost money. Genuine prize surveys publish their rules; TJ Maxx’s monthly $500 drawing, for instance, links its rules from the survey itself.

Is there a DQ survey sweepstakes or mail-in entry?

We found none. Neither start page links sweepstakes rules, so there’s no drawing to enter by mail. If you want surveys with real drawings, LongHorn Steakhouse runs one with a published mail-in route.

Where this Dairy Queen survey information comes from

Both start pages were read on October 28, 2026. DQ can change which site a receipt points to, so the address on your receipt beats this page.

Did your DQ receipt promise something we haven’t covered? A quick email to contact@myreceiptsays.com helps; our editorial policy covers how we check reader tips.

This guide was written by myreceiptsays.com alone; American Dairy Queen Corporation and SMG neither reviewed nor endorsed it. DQ, Dairy Queen and Dilly belong to their owner, and we use the names only to describe the survey. Where DQ’s pages and this guide disagree, DQ’s pages are correct.
