CVSHealthSurvey.com: 17-Digit Survey ID and the $1,000 Prize

CVSHealthSurvey.com: 17-Digit Survey ID and the $1,000 Prize

This CVS Health survey guide follows a CVS receipt invitation and each survey screen, as we went through them ourselves. CVS publishes no prize rules for it, so those are the only two sources.

Not run by CVS. This is a third-party walkthrough, and CVS Health has nothing to do with myreceiptsays.com. Nothing here asks for your Survey ID, receipt or contact details.

CVSHealthSurvey.com: type it, don’t search it

CVS’s survey site isn’t listed in Google, so searching for it brings up other sites instead. The address printed on CVS’s receipt invitation works when you type it straight into your browser’s address bar:

www.cvshealthsurvey.com Go to www.cvshealthsurvey.com

This goes to www.cvshealthsurvey.com, which forwards you to CVS’s survey on survey.medallia.com. Medallia is the survey company whose name appears at the bottom of the survey page. The receipt also has a QR code that does the same job.

A CVS receipt's Share your feedback invitation, with a QR code and the address www.CVSHealthSurvey.com

The CVS Health survey is a short customer feedback survey reached at www.cvshealthsurvey.com, and it starts by asking for the 17-digit Survey ID from your receipt. The receipt invitation and the survey screens we saw ask for your feedback and offer nothing in return: no prize drawing, coupon or ExtraBucks. That makes the “$1,000 CVS sweepstakes” on other guides something we couldn’t confirm.

Is there a $1,000 CVS survey prize?

Not that we could find. CVS’s receipt invitation reads, in full: Share your feedback! We would love to hear about your experience today. This survey will take only minutes and you can take it online. There’s no mention of a prize, a drawing or official rules.

The survey’s first screen thanks you for shopping and asks for the Survey ID, again with no prize. None of the question screens we saw mentions one either, and we found no CVS sweepstakes rules anywhere online. Guides that promise $1,000 don’t link to any rules, and they disagree on whether it’s cash or a gift card.

If your survey does show a sweepstakes entry form, read the official rules linked on that screen: they’re what count. We’d like to see it too, so please send a screenshot to contact@myreceiptsays.com and we’ll update this guide.

What other CVS survey guides claim

We compared the guides that rank for these searches with CVS’s receipt and survey. None of these claims appears in anything from CVS we could find.

What the guide saysWhat CVS’s receipt and survey show
“Win CVS Pharmacy’s $1,000 Monthly Sweepstakes” and a $1,000 gift card (survey.contact, myusasurvey.com)No prize is mentioned, and we found no rules for such a sweepstakes.
“Win $1000 cash” (cwshealthsurvey.shop)Same, and it contradicts the guides that say gift card.
Mail an entry to “16200 Dallas Parkway, Suite 140, Dallas, Texas” (survey.contact, myusasurvey.com)No CVS document we found names this address, so we can’t confirm a card mailed there enters anything.
“Receipt Valid For: 07 Days” (survey.contact)The invitation and survey give no deadline.
“One entry per person per day” (survey.contact)There’s no drawing we could find, so there’s no entry limit to quote.
A coupon worth “$2–$10” for each survey, on a page calling itself an official website (beinghealthylife.com)Nothing we saw offers a coupon, and that site isn’t CVS.

What you need: the CVS Health survey 17-digit Survey ID

You need a CVS receipt with the Share your feedback! invitation on it. The survey’s first screen asks only one thing: Please enter your 17-digit Survey ID. Type all 17 digits exactly as your receipt prints them.

The same screen has a Choose Language / Elija el idioma menu, so you can take the survey in English or Spanish. The receipt says it will take only minutes. CVS doesn’t publish a deadline for taking it after you shop, so do it while the receipt is recent.

How to take the CVS Health survey

  1. Find the Share your feedback invitation on your CVS receipt, with its QR code and the address www.CVSHealthSurvey.com.
  2. Scan the QR code, or type www.cvshealthsurvey.com into your browser’s address bar.
  3. Choose English or Spanish, type your 17-digit Survey ID and select Next.
  4. Check that the store named at the top of the questions is the one you shopped at.
  5. Answer the 0 to 10 questions. The video and written comments are optional, and so are the extra questions offered partway through.
  6. Choose whether to leave a Google review and whether CVS may publish your answers on its website. You can say no to both.

What the survey asks

We went through the survey screen by screen. Most questions use a 0 to 10 scale, and there’s a Back button if you need to change an answer.

  • It names the CVS store and street address it thinks you visited.
  • How likely you are to recommend CVS, and how satisfied you were overall
  • An optional video comment, or a text box if you’d rather type. CVS says a recorded video becomes the property of CVS Health and can’t be made public without your consent.
  • How satisfied you were with the team member at pick-up or checkout, with a box for more comments
  • An offer of a few more questions, about a minute’s worth, including whether you used coupons or ExtraBucks and whether anything made the visit delightful
  • Whether you’d like to leave a Google review, which opens Google in a separate window, and whether CVS may publish your answers anonymously on its website

The text boxes say: We value your privacy; please do not include personal information.

If the CVS survey won’t work

  • Search only shows other sites. Type www.cvshealthsurvey.com into the address bar, or scan the receipt’s QR code.
  • The Survey ID is rejected. Count the digits: there should be 17. Check you haven’t typed another number from the receipt, and that 0 and O, or 1 and I, aren’t mixed up. We can’t see why the survey site turns down a particular ID.
  • The QR code won’t scan. Type the address from the receipt instead.
  • The store named is wrong. The store appears after you enter the Survey ID, so re-check the ID you typed.
  • The video won’t record. Use the written comment box instead; the video is optional.
  • Still stuck. Use the Survey Support link at the bottom of the survey page, or call CVS customer service at 1-800-SHOP-CVS (1-800-746-7287). We can’t fix survey problems from here.

No receipt? There’s nothing to enter by mail

Free mail-in entry exists only where there’s a sweepstakes, and we found no CVS survey sweepstakes or rules naming a mailing address. Don’t post your personal details to an address from an unofficial guide.

There’s no date checker on this page, because CVS doesn’t publish a deadline for taking the survey.

CVS survey scams to watch for

Truth in Advertising reported a phishing email posing as a “CVS Rewards Team” that offered a free gift for a short survey, then asked victims to pay shipping for their “prize”. CVS told the group that legitimate emails about its ExtraCare rewards program come from extracare@your.cvs.com.

Check the address bar: the real survey starts at www.cvshealthsurvey.com and runs on survey.medallia.com. Several guides, some on addresses that look like the real one, call themselves official. They aren’t CVS, and neither is this site. A message saying you’ve won a CVS survey prize doesn’t match anything on CVS’s receipt or survey, and anyone asking for a fee, shipping costs, gift cards or bank details is running a scam. The FTC’s advice: real prizes are free.

CVS Health survey questions

Is there a CVS survey sweepstakes?

Not that we could find. The receipt invitation and the survey screens we saw mention no prize, and we found no official CVS rules. If your survey shows a sweepstakes entry form, the rules linked on that screen are what count.

Is the CVS Health survey $1,000 prize real?

We couldn’t confirm it. Guides promise $1,000 in cash or a gift card, but none links to CVS rules, and they don’t agree on the prize, the deadline or the entry limit.

Where do I take the CVS Health survey?

Type www.cvshealthsurvey.com into your browser’s address bar or scan the QR code on your receipt. The address forwards to CVS’s survey on survey.medallia.com.

What is the CVS Health survey 17-digit Survey ID?

It’s the number the survey’s first screen asks for before any questions. Type all 17 digits exactly as your receipt prints them.

Why doesn’t cvshealthsurvey.com come up on Google?

CVS’s survey site isn’t listed in Google, so search results show other sites. Typing the address from your receipt, or scanning its QR code, is the reliable way in.

Do I get a coupon or ExtraBucks for the CVS survey?

Nothing we saw offers one. The survey asks whether you used coupons or ExtraBucks on your trip, but it doesn’t promise any for taking part.

How long do I have to take the CVS survey?

CVS doesn’t publish a deadline in anything we could find. Take it while your receipt is recent.

Can I take the CVS survey in Spanish?

Yes. The first screen has a Choose Language / Elija el idioma menu.

Do I have to record a video?

No. You can type your comments instead. CVS says a recorded video becomes the property of CVS Health.

Will CVS publish my survey answers?

Only if you agree. Near the end, the survey asks whether CVS may display your responses anonymously on its website, and you can decline.

How do I contact CVS about the survey?

CVS lists customer service at 1-800-SHOP-CVS (1-800-746-7287). The survey page also has a Survey Support link at the bottom.
